Transition

Transition is when young people prepare to move from children's services to adult services. It also includes the time at the start of being under adult services.

Most people say transition starts around 11 years of age and finishes around 25 years of age.

Ready Steady Go is a transition framework that was developed by Dr Arvind Nagra, a children's kidney doctor in Southampton. It is designed for young people with any long-term health condition, to help them prepare for moving to adult services.

Aspire is a programme developed by Donna Hilton, Youth Work Manager at Nottingham Children's Hospital, to help young people with transition by achieving specific competencies.
