Ready Steady Go is a transition framework that was developed by Dr Arvind Nagra, a children's kidney doctor in Southampton.  It is designed for young people with any long-term health condition, to help them prepare for moving to adult services. 

It is in the form of questionnaires that cover understanding of the health condition, the treatment and how it affects different aspects of the young persons life. 

Ready is designed to be used from about 13 years old.  It is red in colour like a traffic light. 

Steady is for slightly older teenagers.  It covers similar issues but in more depth. It is amber, like the traffic light that means "get ready"

Go is green and meant for young people very close to moving to adult services. 

On-line forms can be found here.  They can be printed off and brought to your appointments to discuss with your doctor, nurse or other key worker.
